Forms

  • Effective October 15, 2018, all new protocols must be submitted electronically via GW iRIS. Until that date, new protocol submissions may be submitted either using the forms below or via GW IRIS.
  • Effective November 1, 2018, all modifications, renewals and closures must be submitted electronically via GW iRIS. Until that date, modifications, renewals and closures may be submitted either using the forms below or via GW iRIS.

Submissions via GW iRIS

To begin a new electronic submission, login to GW iRIS and choose “Study Assistant” and “Add a New Study”

Submit a new protocol, modification, renewal or closure via GW iRIS
